Long Term Care


Long-term Care Our 70-bed, skilled nursing facility provides 24-hour quality nursing care to residents. The nursing staff includes RNs, LPNs, TMAs and registered nursing assistants. Other services available include dietary, laundry, housekeeping, beauty shop and social services. Also available daily are physical therapy, occupational therapy and speech therapy. Geriatic psychological services are available on a regular basis. Our staff strives to meet all the physical, mental, spiritual and phycho-social needs of our residents. Our goal is to assist each resident to function at their highest level of wellness and independence.

Adult Day Services
Adult day services are available in the nursing home with application made through the social worker. Information on payment, hours, and services are available on request.


Wagon ride Activities
The activity department provides a wide variety of activities that keep the residents active and involved. The golf cart and wagon is one of the favorite summer activities that help residents get out into the community and enjoy the outdoors.

Social Services The social worker is available for assistance with discharge planning, such as making referrals to a home health agency, setting up "Meals Each Day," or acquiring needed equipment for your return home. Discharge Planning can also mean finding a nursing home placement. The social worker can also help you make contact with the county in order to apply for medical assistance when needed. In addition, she can discuss with you financial needs regarding continued care, as well as your financial standing and options. Swing bed and respite bed stays will also involve the social worker, who will take a social history and help explain these two types of care and their availability.

The social worker is also available to assist you in completing advance directives such as Health Care Directives. Health Care Directive forms are also available from the social worker if you would prefer to complete it on your own.

The social worker is available during office hours and also as needed for information or referrals. Upon a patient's request, the social worker will visit the patient as time allows and try to assist him/her with their needs. Please call 320-289-2400, ext. 341, to contact the social worker.


Advance Directives
Advance directives are merely written statements of a patient's wishes regarding health care decisions, and allow you to designate a legal agent to make decisions in your place in the event of incapacity.
