Director of Nursing

Position - Care Center

The Director of Nursing participates as a member of the AAH management team as a Senior Leader in planning, policy formulation, and administrative decision making.  The Director of Nursing responsibilities include: 

  • Plans, organizes, directs, and maintains the operation of the long-term care facility and oversees the management of  the dietary, activities, social services and home care departments.

  • Develops and maintains facility and departmental philosophy, goals, objectives, standards of practice, and quality of care.

  • Participates in facility/departmental strategic planning and decision making and keep current and implement new practices as needed.

  • Completes an annual capital and operating budgets for care center nursing departments and oversees direct reports financial statements.

  • Ensures that nursing care policies, practices and procedures, and nursing education and training are established and consistent with nursing standards of care and regulatory requirements.

  • Participates in quality initiatives and risk management by providing input and ensuring improvement in quality of care and safety for residents and staff; and strives for ways to improve overall activities and functions of the care center and the facility.

  • Protect and promote residents rights and choices.

  • Directs, evaluates, and delegates management of personnel ensuring optimal utilization of time and talents in providing quality care.

  • Participates and conducts meetings, in-services, and discussions to promote efficient and effective delivery of services and quality of care.

  • Participates in recruitment and selection of personnel including assignments, orientation and supervision. Supervise, coach/mentor, evaluate, and discipline personnel.

  • Maintain standards of regulatory agencies (CMS, MDH ,OSHA, OHFC) and submit required reports.

  • Be available to residents and families as a liaison and follow through in appropriate manner and to promote good public relations with residents, families, visitors, volunteers and community.

  • Works collaboratively with others to accomplish goal/task and maintain interdepartmental relations to ensure quality and continuity of care.

  • Maintain compliance with regulations for Skilled Nursing Facility.

Education/Experience/Credentials
  • Graduate of an accredited school of nursing and licensed as a Registered Nurse in the State of Minnesota, Bachelor’s or graduate degree in nursing or administration desirable.

  • Minimum of two years of direct experience in geriatric nursing.

  • Supervisory experience preferred.

Food Service Manager

Position - Food Service Manager (Food Service)

Appleton Area Health is currently hiring a Certified Dietary Manager.  The primary purpose of this position is to oversee the daily operations of food-service department, provide leadership and guidance to ensure that food quality, safety standards, and client expectations are met.

Education/Experience/Credentials

  • Graduate of Dietary Manager’s Program

  • 2-yr, or 4-yr food-service program

  • Successful completion of Certified Dietary Manager exam

  • Two years’ experience in food-service management

  • Prior experience in healthcare food-service preferred

LPN/RN - Charge Nurse

Position - Licensed Practical Nurse/Registered Nurse (Care Center)

Appleton Area Health is currently hiring a Registered Nurse or Licensed Practical Nurse to work in the Care Center as a full-time Day Shift Staff/Charge Nurse.  The primary purpose of this position is to assist with and ensure implementation of individualized Resident care plans in order to meet the physical, mental, psycho-social, and spiritual needs of Residents in accordance to state & federal regulations & AAH policies and procedures.

Education/Experience/Credentials

·        Must be, as a minimum, a graduate from a LPN training program and possess a current license in the state of Minnesota.

·        ACLS certification (or willingness to obtain)

·        CPR certification (or willingness to obtain)

Trained Medication Aide (TMA)

Position - Care Center

The primary purpose of your position, as a Trained Medication Aide, is to administer medications and complete documentation under the direct supervision of a licensed nurse.

Education/Experience/Credentials

  • Must have a Minnesota Nursing Assistant Certification in good standing

  • Unlicensed personnel must have completed a medication administration training program offered through a Minnesota postsecondary educational institution

  • Knowledge of medication administration, medical terminology, medications, and diagnostic services

  • Ability to answer phones and relay messages in a professional manner

  • Possess a calm, courteous, and friendly attitude

  • Previous experience as a CNA or TMA preferred

  • Must enjoy working with the geriatric population

Housekeeping - Apple Ridge Estates

Position - Housekeeping

Housekeeping staff perform a variety of general cleaning tasks to maintain patient room, offices, hallways and other assigned areas of Appleton Area Health Services.

Education/Experience/Credentials

  • May have less than a high school education

  • No experience necessary

  • On-the-job training will be provided

Unlicensed Assistive Personnel (UAP)

The primary purpose of your position, as a UAP, is to provide personal care and delegated nursing services designed to maintain the clients’ physical and emotional well-being.

Certified Nursing Assistant

Position - Care Center (Day shift)

The primary purpose of your position, as a nursing assistant, is to assist residents with activities of daily living in order to meet their physical, mental, psycho-social, and spiritual needs.  The care you provide is in accordance with the resident’s individualized plan of care and established policies and procedures.

Education/Experience/Credentials
  • Must have a Minnesota Nursing Assistant Certification (if you are interested in this position but not yet certified, AAH is willing to train/certify candidates after completion of hiring process)

Dietary Aide

Appleton Area Health is currently hiring a Food Service Aide to assist in food preparation and delivery of meals to residents and hospital patients, documentation of meal and fluid intakes, dish washing and departmental cleaning; Ensure and adhere to departmental policies and procedures, follow sanitation guidelines, and other duties as assigned.

  • Must be able to work productively and positively with fellow employees and Dietary Manager

  • Work well with others

  • Requires working around heat from stove/oven and steam from food/steamer

  • Requires frequent standing and walking.
