Director of Nursing

Position - Care Center

The Director of Nursing participates as a member of the AAH management team as a Senior Leader in planning, policy formulation, and administrative decision making.  The Director of Nursing responsibilities include: 

  • Plans, organizes, directs, and maintains the operation of the long-term care facility and oversees the management of  the dietary, activities, social services and home care departments.

  • Develops and maintains facility and departmental philosophy, goals, objectives, standards of practice, and quality of care.

  • Participates in facility/departmental strategic planning and decision making and keep current and implement new practices as needed.

  • Completes an annual capital and operating budgets for care center nursing departments and oversees direct reports financial statements.

  • Ensures that nursing care policies, practices and procedures, and nursing education and training are established and consistent with nursing standards of care and regulatory requirements.

  • Participates in quality initiatives and risk management by providing input and ensuring improvement in quality of care and safety for residents and staff; and strives for ways to improve overall activities and functions of the care center and the facility.

  • Protect and promote residents rights and choices.

  • Directs, evaluates, and delegates management of personnel ensuring optimal utilization of time and talents in providing quality care.

  • Participates and conducts meetings, in-services, and discussions to promote efficient and effective delivery of services and quality of care.

  • Participates in recruitment and selection of personnel including assignments, orientation and supervision. Supervise, coach/mentor, evaluate, and discipline personnel.

  • Maintain standards of regulatory agencies (CMS, MDH ,OSHA, OHFC) and submit required reports.

  • Be available to residents and families as a liaison and follow through in appropriate manner and to promote good public relations with residents, families, visitors, volunteers and community.

  • Works collaboratively with others to accomplish goal/task and maintain interdepartmental relations to ensure quality and continuity of care.

  • Maintain compliance with regulations for Skilled Nursing Facility.

Education/Experience/Credentials
  • Graduate of an accredited school of nursing and licensed as a Registered Nurse in the State of Minnesota, Bachelor’s or graduate degree in nursing or administration desirable.

  • Minimum of two years of direct experience in geriatric nursing.

  • Supervisory experience preferred.

Unlicensed Assistive Personnel (UAP)

Position - Apple Ridge Assisted Living

The primary purpose of your position, as a UAP, is to provide personal care and delegated nursing services designed to maintain the clients’ physical and emotional well-being.

Education/Experience/Credentials

  • High school diploma or GED preferred

Trained Medication Aide (TMA)

Position

The primary purpose of your position, as a Trained Medication Aide, is to under the direct supervision of a licensed nurse, administer medications and complete documentation.

Education/Experience/Credentials
  • Must have a Minnesota Nursing Assistant Certification in good standing

  • Unlicensed personnel must have completed a medication administration training program offered through a Minnesota postsecondary educational institution

  • Knowledge of medication administration, medical terminology, medications, and diagnostic services

  • Ability to answer phones and relay messages in a professional manner

  • Possess a calm, courteous, and friendly attitude

  • Previous experience as a CNA or TMA preferred

  • Must enjoy working with the geriatric population

Rehab Coordinator

 

Appleton Area Health is currently seeking an individual to collaborate with RN supervisor, RN Case Managers, Assistant Director of Nursing, and the Director of Nursing to implement, organize, and sustain a successful Restorative program in the Care Center.  The job will demand a high level of energy, a positive attitude, and compassionate care. Employee is responsible for:

  • Ensuring rehab exercise and range of motion activity are completed on a daily basis.

  • Documenting, and reporting pertinent data to RN. 

  • Documenting resident assessments and for routine monitoring with oversight of an RN.

  • Development of personalized care plans relating to each individual participating in the rehab program with the oversight of an RN. 

Education/Experience/Credentials

  • Knowledge and interest in exercise, physiology, and kinesiology.

Housekeeping/Laundry Aide

Position - Environmental Services

Housekeeping staff perform a variety of general cleaning tasks to maintain patient room, offices, hallways and other assigned areas of Appleton Area Health Services.

 

Laundry staff performs washing and drying of long term residents, in patients, and other essential laundry used by the facility. May be asked to do other tasks as assigned by manager.

Education/Experience/Credentials
  • Maintains established AAHS standards, departmental policies, performance improvement program, safety, management of information, and infection control standards.

  • Utilizes excellent customer service skills at all times.

  • Complies with federal and state regulations and accrediting and licensing agencies at all times.

  • Work well with others

  • May have less than a high school education, no experience required on-the-job training will be provided.

EMT

Position - Ambulance Service

Appleton Area Health is currently seeking Emergency Management Technicians. Applicants must be at least 18 years of age as well as hold a current EMT Certification in good standing or a willingness to complete.

Licensed Practical Nurse/Certified Medical Assistant

Position - Clinic

Appleton Area Health is currently seeking a Licensed Practical Nurse or Certified Medical Assistant to work in the Clinic assisting providers with patient care taking patient vital signs, collecting blood and other specimens, and giving vaccinations and injections.  The LPN/CMA will perform miscellaneous clinic duties in connection with referrals, appointment scheduling, management of patient medical records, and ordering medical and office supplies.

Education/Experience/Credentials
  • Must be, as a minimum, a graduate from a LPN training program and possess a current license in the state of Minnesota or a graduate from a Medical Assistant Program and possess and certificate in good standing.

  • CPR certification (or willingness to obtain)

Licensed Practical Nurse/Registered Nurse

Position - Care Center

Appleton Area Health is currently seeking a Licensed Practical Nurse or Registered Nurse to work in the Care Center as a charge nurse. The primary purpose of this position is to assist with and ensure implementation of individualized Resident care plans in order to meet the physical, mental, psycho-social, and spiritual needs of Residents in accordance to state & federal regulations & AAH policies and procedures.

Education/Experience/Credentials

  • Must be, as a minimum, a Registered Nurse and possess a current license in the state of Minnesota or a graduate from a LPN training program and possess a current license in the state of Minnesota.

  • CPR certification (or willingness to obtain)

  • ACLS certification (or willingness to obtain)

Certified Nursing Assistant

Position - Care Center (Day shift)

The primary purpose of your position, as a nursing assistant, is to assist residents with activities of daily living in order to meet their physical, mental, psycho-social, and spiritual needs.  The care you provide is in accordance with the resident’s individualized plan of care and established policies and procedures.

Education/Experience/Credentials
  • Must have a Minnesota Nursing Assistant Certification (if you are interested in this position but not yet certified, AAH is willing to train/certify candidates after completion of hiring process)
